  -Insert your reception No. (Ex. recepetion No.:09-500 Type 09500)
  -You will shortly receive your test report in PDF format.
• Via e-mail address (declare your email address while being received)
• Sending Report to your physician (declare while being received)
• Via Fax
• Via Jordan web site

Please notice that in order to have your test report you may have no debt record
